What is the correlation between bananas and latex allergy

Natural rubber latex is known to cause allergic reactions of Type I (e.g. anaphylaxis) and Type IV (e.g. allergic contact dermatitis) as well as non-allergic irritant contact dermatitis.

Irritant contact dermatitis

Natural rubber latex can also cause irritant contact dermatitis,[10] a less severe form of reaction that does not involve the immune system. Contact dermatitis causes dry, itchy, irritated areas on the skin, most often on the hands.[11] Latex-glove induced dermatitis increases the chance of hospital-acquired infections, including blood-borne infections, being transmitted.[12]

Type IV (allergic contact dermatitis)

Type IV allergy, also known as allergic contact dermatitis, involves a delayed skin rash that is similar to poison ivy with blistering and oozing of the skin (see urushiol-induced contact dermatitis).

It can be diagnosed through a positive skin patch test, although a negative test does not law out a latex allergy.[8] Severe irritation takes put if a latex catheter is inserted in the urinary tract of a person allergic to latex. That is especially severe in case of a radical prostatectomy due to the open wound there and the exposure lasting e.g. two weeks. Intense pain may indicate such situation.[9]

Type I

The most serious and rare form of latex allergy, Type I hypersensitivity can cause an immediate and potentially life-threatening reaction, not unlike the severe reaction some people own to bee stings.

Such reactions account for a significant proportion of perioperativeanaphylactic reaction, especially in children with myelomeningocele.[4][5] Type I natural rubber latex allergy is an IgE (immune) mediated reaction to proteins found in the Hevea brasiliensis tree, a type of rubber tree.

Testing for type I natural rubber latex allergy is through blood testing to determine if the patient is producing IgE antibodies to latex proteins.

Anaphylactic shock can be provoked in allergic persons by the previous use of latex in an area: latex is typically powdered to prevent sticking, latex proteins become attached to the particles of powder, and the powder becomes airborne when the latex item is used, triggering potentially life-threatening Type I reactions when the latex-contaminated powder is inhaled by susceptible persons.[6] In radiological units latex allergy may mimic contrast medium allergy[7]

Latex-fruit syndrome

People who own latex allergy also may own or develop an allergic response to some plants and/or products of these plants such as fruits.

This is known as the latex-fruit syndrome.[13] Fruits (and seeds) involved in this syndrome include banana, pineapple, avocado, chestnut, kiwi fruit, mango, passionfruit, fig, strawberry, papaya, apple, melon, celery, potato, tomato, carrot, and soy. Some, but not every of these fruits contain a form of latex. Hevein-like protein domains [14] are a possible cause for allergen cross-reactivity between latex and banana[15] or fruits in general.[16]

Natural rubber latex contains several conformational epitopes located on several enzymes such as Hev b 1,[17]Hev b 2,[18]Hev b 4,[19]Hev b 5[20] and Hev b .[21][22]

  • What are latex allergy symptoms?

    In most cases, latex allergy develops after numerous previous exposures to latex. Latex allergy symptoms may include hives, itching, stuffy or runny nose. It can cause asthma symptoms of wheezing, chest tightness and difficulty breathing. Symptoms start within minutes after exposure to latex containing products. The most severe latex allergy can result in anaphylaxis, a serious allergic reaction involving severe breathing difficulty and/or drop in blood pressure (shock).

    Allergic skin problems can happen following direct contact with allergic latex proteins in latex glove products.

    Symptoms may include immediate itching, redness and swelling of skin that touched the item containing latex. These and other latex allergic reactions are less common now. Numerous hospitals or doctors’ offices own switched to non-latex gloves or low protein latex gloves.

    A second type of skin allergy called “allergic contact dermatitis” may be caused by chemicals used to manufacture rubber gloves. This dermatitis is recognized by the eczema and blisters on the back of the hands. It resembles a poison ivy rash, and begins 1 to 3 days after wearing rubber gloves.

    Direct physical contact with latex products is not needed to trigger an allergic reaction. Anaphylaxis and severe asthmatic reactions own been caused by inhaling latex proteins in the air resulting from the powder in the latex glove.

  • What are latex allergy symptoms?

    In most cases, latex allergy develops after numerous previous exposures to latex. Latex allergy symptoms may include hives, itching, stuffy or runny nose. It can cause asthma symptoms of wheezing, chest tightness and difficulty breathing.

    Symptoms start within minutes after exposure to latex containing products. The most severe latex allergy can result in anaphylaxis, a serious allergic reaction involving severe breathing difficulty and/or drop in blood pressure (shock).

    Allergic skin problems can happen following direct contact with allergic latex proteins in latex glove products. Symptoms may include immediate itching, redness and swelling of skin that touched the item containing latex. These and other latex allergic reactions are less common now. Numerous hospitals or doctors’ offices own switched to non-latex gloves or low protein latex gloves.

    A second type of skin allergy called “allergic contact dermatitis” may be caused by chemicals used to manufacture rubber gloves.

    This dermatitis is recognized by the eczema and blisters on the back of the hands. It resembles a poison ivy rash, and begins 1 to 3 days after wearing rubber gloves.

    Direct physical contact with latex products is not needed to trigger an allergic reaction. Anaphylaxis and severe asthmatic reactions own been caused by inhaling latex proteins in the air resulting from the powder in the latex glove.

  • What triggers the allergic reaction to latex?

    When people with latex allergy come into direct contact with latex, an allergic reaction may follow. Common examples include:

    1. A medical or dental procedure conducted by health care workers wearing natural rubber latex gloves
    2. Blowing up a rubber balloon

    What foods are potential problems for people with latex allergy?

    If you own latex allergy you also can own food allergies.

    The foods most likely to cause this problem include: apple, avocado, banana, carrot, celery, chestnut, kiwi, melons, papaya, raw potato and tomato.


    Epidemiology

    Estimates of latex sensitivity in the general population range from % to %.[28]


    Risk factors

    1. Industrial rubber workers, exposed for endless periods to high amounts of latex. About 10% develop an allergic reaction.[12]
    2. Children with spina bifida. Up to 68% will own a reaction.[24]
    3. Health care providers. Given the ubiquitous use of latex products in health care settings, management of latex allergy presents significant health organizational problems.

      Those healthcare workers—such as physicians, nurses, aides, dentists, dental hygienists, operating room employees, occupational therapists, laboratory technicians, and hospital housekeeping personnel—who frequently use latex gloves and other latex-containing medical supplies are at risk for developing latex allergy.[25] Between about 4% to 17% of healthcare workers own a reaction, which generally presents as Irritant Contact Dermatitis. This contact dermatitis can develop further through allergic sensitivity to a status of full anaphylactic shock.

      Apart from the uncomfortable and in some cases life-threatening health implications, this will effectively hinder the person from working with any quantity of latex and could impede their chance of maintaining their vocation.[26] In the surgical setting, the risk of a potentially life-threatening allergic reaction by a patient has been deemed by Johns Hopkins Hospital to be sufficiently high to replace every latex surgical gloves with synthetic alternatives.[27]


    Cross-reactivity with banana

    Mäkinen-Kiljunen studied 47 patients to investigate banana allergy in patients with latex allergy in , measuring latex-, banana-, and pollen-specific (birch, timothy, and mugwort) IgE.

    Thirty-one patients were also given skin-prick tests with banana and were queried about reactions after consuming bananas. Of the 47 sera samples, latex RAST results were positive in 31 and banana RAST results in RAST results from latex and banana were correlated (25 of the 31 latex RAST-positive samples were also banana RAST-positive), but not with pollen. Sixteen of the 31 patients who ate banana reported symptoms, and 11 of the 31 patients given the banana skin-prick test showed positive results.

    The author confirmed the cross-reactivity of IgE antibodies for latex and banana, identifying for the first time a structurally similar antigen/allergen as at least one antigen from banana fused with an antigen from latex in crossed-line immunoelectrophoresis.6

    In , Mikkola et al. investigated whether proteins similar to hevein, a major natural rubber latex allergen, are present in banana and account for cross-reactivity between these botanicals. Immunoblotting revealed that 9 of 15 sera from latex-allergic patients with IgE to hevein also bound to and kd banana proteins. Studies using ELISA [enzyme-linked immunosorbent assay] showed that the common presentation of hypersensitivity to banana among patients allergic to latex could be attributed to cross-reacting IgE antibodies binding to epitopes in hevein and in the then-newly identified hevein-like endochitinase found in banana.7

    Latex, the sap of the Hevea brasiliensis tree, has been used in the manufacture of medical equipment since when the first surgical rubber gloves were manufactured.

    The possibility of severe latex (natural rubber) allergy in individuals with spina bifida was first raised in Since that time research studies own shown that between 18 and 73% of children and adolescents with spina bifida are sensitive to latex as measured by history or blood test. The type of allergic reaction experienced can range from watery and itchy eyes and/or sneezing and coughing, to hives (a blotchy, raised, itchy, rash) to swelling of the trachea (windpipe) and even to life-threatening changes in blood pressure and circulation (anaphylactic shock).

    Although the cause of rubber allergy in individuals with spina bifida is not known, it is theorized that sensitization may happen from the early, intense, and constant exposure to rubber products through multiple surgeries, diagnostic tests and examinations, also from bladder and bowel programs.

    Exposure to latex can happen when products containing rubber come in contact with a persons skin or mucous membranes such as the mouth, eyes, genitals, bladder or rectum.

    Serious reactions can also happen when latex enters the bloodstream. Some cases of severe reaction following injection of medication through latex stoppers, IV ports or syringes own been reported. In addition, the powder from balloons or gloves can absorb particles and become airborne causing reactions when breathed by a latex sensitive person. Food that has been handled by latex gloves may also cause a reaction, and people who own allergic reactions to latex may also be allergic to certain foods, including bananas, chestnuts, avocados and kiwi fruit.

    Although a lot has been learned about rubber allergy in the final few years, the problem is really just beginning to be studied and understood.

    Interestingly, health care workers such as nurses, doctors, and dentists, individuals born with severe bladder abnormalities, and some people who own had multiple surgical procedures are also at increased risk for rubber allergy although their risk is much lower than it is for individuals with spina bifida.

    The Food and Drug istration and the Centers for Disease Control are investigating the problem of rubber allergy and current efforts are aimed at finding the component(s) of latex responsible for causing this allergy, developing methods of producing safe, non allergy causing rubber, and labeling products indicating natural rubber (latex) content.

    Since the tale of rubber allergy is just beginning, and, without a doubt new understanding will emerge over the next several years, recommendations made now may need to be revised.

    Because of the life-threatening nature of this allergy a list of current recommendations is included. Individuals with spina bifida and their families are urged to consider the following recommendations and to discuss them with members of their health care team.

    All individuals with spina bifida should be considered at high risk for having an allergic reaction to rubber and should avoid contact with rubber products, particularly during medical or surgical procedures.

    Only non-latex gloves and catheters should be used. Alternative products, generally made of silicone, plastic or vinyl, can be safely substituted.
    Individuals who own experienced allergic reactions during surgical or medical procedures should consider wearing a medic-alert bracelet or necklace, carrying auto-injectable epinephrine and sterile non-latex gloves for emergency use, and discussing latex allergy with every health care and community providers including school, day care and camp. In addition, consultation with an allergist familiar with the problem is recommended to fully assess the risks and the possible need for preoperative treatment with special medications to suppress the potential for severe allergic reaction.

    Avoidance of every latex-containing items, especially in the operating room, is strongly recommended.
    The following commonly encountered items may contain latex and pose a risk to the latex sensitive individual: Healthcare items which may contain latex: gloves, catheters, tourniquets, elastic bandages, ace wraps, I.V. tubing injection ports, medication vials, adhesive tape, bandaids.

    Home/community items which may contain latex: balloons, pacifiers, dental dams, rubber bands, elastic in clothing, beach toys, Koosh balls, baby bottle nipples, condoms, diaphragms, diapers, art supplies.

    Please note that this is only a partial list and it is strongly recommended that individuals with spina bifida and their families enquire about the composition of products used in their care.

    Latex allergy is a medical term encompassing a range of allergic reactions to the proteins present in natural rubber latex.[1] Latex allergy generally develops after repeated exposure to products containing natural rubber latex.

    When latex-containing medical devices or supplies come in contact with mucous membranes, the membranes may absorb latex proteins. The immune system of some susceptible individuals produces antibodies that react immunologically with these antigenic proteins.[2] As numerous items contain or are made from natural rubber, including shoe soles, elastic bands, rubber gloves, condoms, baby-bottle nipples, and balloons, there are numerous possible routes of exposure that may trigger a reaction. People with latex allergies may also own or develop allergic reactions to some fruits, such as bananas.[3]


    Alternatives

    1. Products made from guayule natural rubber emulsions also do not contain the proteins from the Hevea rubber tree, and do not cause allergy in persons sensitized to Hevea proteins.[24][30][31]
    2. Chemical treatment to reduce the quantity of antigenic proteins in Hevea latex has yielded alternative materials, such as Vytex, which reduce exposure to latex allergens while otherwise retaining the properties of natural rubber.
    3. Synthetic rubbers such as elastane, neoprene, nitrile and artificially synthesized polyisoprene latex do not contain the proteins from the Hevea brasiliensis tree.[29]
    4. In , the first polyurethane condoms were produced, designed for people with latex allergies.

    For some people, the sensitivity is so extreme that replacement of latex products with products made from alternative materials may still result in a reaction if the products are manufactured in the same facility as the latex-containing products, due to trace quantities of natural rubber latex on the non-latex products.[31]


    Research substantiates reports

    Blanco et al.

    conducted a prospective study in their outpatient clinic in 25 patients diagnosed with latex allergy, published in They used a clinical questionnaire, skin-prick tests, skin test with a latex extract, and identification of entire and specific IgE to assist ascertain clinical characteristics and cross-reactivity. Of the 23 women and 2 men in the study (mean age 33, plus or minus 9 years), 9 (36%) experienced latex-induced reactions characterized by systemic anaphylaxis.

    In 13 patients (52%), 42 food allergies were identified, and 23 included systemic anaphylaxis. Avocado (9), chestnut (9), banana (7), kiwi (5), and papaya (3) were the most common foods to cause hypersensitivities. The researchers concluded that their little study supported the reality of a “latex-fruit syndrome.”3

    Another study aimed to characterize the cross-reactivity of latex and foods and assess clinical significance. Beezhold et al. examined 47 patients allergic to latex and 46 nonallergic controls. The investigators found immunologic reactivity to foods to be prevalent (33 latex-allergic patients and seven controls), with 27% of food skin-prick tests positive in the latex-allergic group.

    In addition, clinical symptoms were linked to 27% of positive skin-prick tests. Among the 17 patients who displayed clinical allergies to at least one food, 14 showed local sensitivity reactions, with anaphylaxis noted in Avocado (53%), potato (40%), banana (38%), tomato (28%), chestnut (28%), and kiwi (17%) were the foods most frequently cited for provoking a skin test reaction. The authors observed extensive cross-reactivity between latex sensitivity and specific foods, with potatoes and tomatoes reported for the first time.4

    In , Brehler et al.

    studied serum samples from patients whose immediate hypersensitivity to latex proteins was clinically observable and documented. The samples were assessed for IgE antibodies against several fruits, with fruit-specific IgE antibodies recorded in %. Radioallergosorbent (RAST) -inhibition tests yielded the recognition of cross-reacting IgE antibodies in latex and multiple fruit allergens: avocado, banana, chestnut, fig, kiwi, mango, melon, papaya, passion fruit, peach, pineapple, and tomato. The investigators recorded intolerance reactions and noted that % of their patients reported allergic symptoms after consuming these fruits. Fruit-specific IgE antibodies were detected in only % of these patients, suggesting to the researchers that serologic tests were suboptimal in forecasting food hypersensitivities in patients who are allergic to latex.5
